POC Minimum Requirements
Policy
For each patient seen at our rehabilitation agency, a Plan of Care will be established that will include all required documentation as determined by regulatory guidelines and standards of practice. At a minimum, the POC must include objective and measurable treatment goals. When therapy services are indicated, the type, amount, frequency and duration of therapy is to be documented.
