Indexing

Policy

A clinical record will be created for each patient who receives services at our facility. Clinical records will be systematically indexed and organized to facilitate the acquisition and retrieval of records.   


Records are electronically maintained in our EMR System and can be retrieved by authorized users through simple search using patient name, patient date of birth or system-created ID number.  For each patient, admissions are organized using a Case Identification Name which consists of the discipline (PT/OT/ST and the month/year of referral.  For example, a patient referred for physical therapy in November 2022 will have a case ID of PT 112022.


Electronic software is used to receive and file information including e-Fax, File Center, and Adobe Professional.
