Communication Plan

Policy

Consistent with applicable regulations, LifeCare will maintain a written Communication Plan that defines how our facility will coordinate patient care within the facility, across healthcare providers, and with state and local public health departments in the event of a disaster or emergency whether natural, man-made or isolated to our facility. This plan will include how our facility will interact and coordinate with emergency management agencies and systems to protect patient health and safety. The Communications Plan will:


Include names and contact information for:

  • Staff

  • Entities providing services under arrangement

  • Physicians 

  • Other facilities

  • Volunteers if used

  • Other Providers/Suppliers of the same Medicare type


Contact information for:

  • Federal, state, tribal, regional and local emergency preparedness staff

  • Other sources of assistance


Primary and alternate means of communicating with staff and federal, state, tribal, regional, and local emergency management agencies.


A plan for sharing information and medical documentation with other providers to maintain continuity of care.


A means of providing information about facility needs and/or our ability to assist.
